Elderly Americans represent a large and growing segment of our communities. When faced with tough decisions regarding long-term care, these individuals and their families need an easier way to get information about outreach and screening services, assessment and service coordination, and how community and institutional settings can help.
As members of Avalon's Care Coordination and Case Management team, we provide up-to-date information and counseling regarding available long-term care service options, health promotion, disease prevention and community resources. All information addressing explanations of options, decision points and actions to take is delivered in a clear, understandable way. We also offer, referral intake and case tracking.
State governments and health organizations are increasingly interested in helping their constituents or members - especially the elderly, the disabled and their families - make well-informed choices about healthcare. Avalon helps them do this. In addition, we enable states to identify those who are most at risk of acute care or nursing home placement, and provide effective follow-up and intervention to allow those individuals to remain in their home or assisted living environments.
